根据外交部通知，中国驻美国大使馆签证处自2007年9月1日起将正式启用新的《中华人民共和国签证申请表》，旧签证申请表同时停止使用。签证处提醒申请人及代办机构，在递交签证申请时请务必使用新的签证表，旧签证申请表将不被受理。谢谢您的合作。

Notice: According to the instruction from Beijing, effective from September 1, 2007, the new Visa Application Form of the People’s Republic of China will be used and old versions will be void and discontinued. Please be advised that applicants and visa agencies use new form when submitting visa application, old form is unacceptable and will be returned. Thanks for your cooperation.
